Government Contributions

There are a couple forms of government funding options that are provided for in home care(Support at Home). Legacy Aged Care is an approved provider to manage and deliver Home Care Packages(Support at Home).

 

Home Care Package (Support at Home)

A Home Care Package(Support at Home) is one form of aged care funding provided by the Australian Government. It helps to pay for services that enable you to remain independent, safe, and well at home.
We help you select services that are tailored to your individual needs and ensure you get the most value out of your Home Care Package(Support at Home).

 

Contributions Explained

Depending on your situation, you may be eligible for a Home Care Package(Support at Home) ranging from levels 1-8. The difference between the levels is the amount of support you require to meet your needs and the amount of government contributions provided to deliver these support services.

    • in person or via telehealth
    • individually or in a group-based format (e.g., clinically supervised group exercise classes)
  • A treatment program may be delivered directly or implemented by an allied health assistant or aged care worker under supervision where safe.
  • Prescribing and follow-up support for Assistive Technology and Home Modifications
  • Subsidised through other programs:
    • other government programs must be accessed in first instance (e.g., Chronic Disease Management Plan, Mental Health Plan)

Support may include:
    • Assistance to access translating and interpreting services and translation of information into the older person's chosen language
    • Referral pathways to advocacy or community organisations

  • Engagement of a diversional therapist to design and/or facilitate recreation programs that promote social, psychological and physical well-being for older people who live with age or disability related impairments that will benefit from a tailored program to enable and maintain participation.
  • Subsidised through other programs:
    • other government programs must be accessed first (e.g., Chronic Disease Management Plan)
    • services more appropriately funded through the primary health care system (e.g., ambulance and hospital costs, medical diagnosis and treatment, medicine dispensing, psychiatry, dental care)
    • management of conditions unrelated to age/disability related decline (e.g., acute mental health)
    • services from a Chinese Medicine Practitioner, such as herbal medicine dispensing, are out of scope for aged care (see description for acupuncture exception).

    • gardening services that relate to visual appeal rather than safety/accessibility (e.g., installation and maintaining plants, garden beds and compost).
    • professional maintenance and repair services that would usually be paid for (e.g., professional pest extermination, installing cabinetry, replacing carpets due to usual wear and tear) except if there is an imminent age- related safety risk (e.g., repairing uneven flooring that poses a falls risk or section of carpet damaged by a wheelchair)
    • services that are responsibility of other parties (e.g., landlords, government housing authorities, generally covered by private insurance).

Participant contributions

From 1 November 2025, participants in the Support at Home program will contribute towards the cost of some services, while other services will be free of charge. You will only pay contributions for services you receive.

Contribution rates

Contribution rates will be based on your income and assets and vary by the amount and type of service you have received. An income and asset assessment similar to the Age Pension means-test will be used to determine your contribution rate. Those on the Age Pension and Commonwealth Seniors Health Card holders will pay lower contributions than self-funded retirees.

The amount you will pay will be set as a percentage of the price of each service you have received. This means that when you pay a contribution, the government will pay the remainder to the provider.

The contribution rate will be based on the type of Support at Home service received. The table below shows that:

Everyday living services, such as cleaning and gardening, will attract the highest contribution rates.

Grandfathered Clients

Grandfathered HCP recipients are those who, on 12 September 2024, were either receiving a Home Care Package(Support at Home), on the National Priority System, or assessed

as eligible for a package.

Grandfathered participant contribution arrangements include:

  • Previous HCP clients who were not required by Services Australia to pay an income-tested care fee will continue to make no contributions for the remainder of their time in Support at Home. This group includes all grandfathered full-rate pensioners.
  • Previous HCP clients who, based on the outcome of their income test, were required to pay an income-tested care fee, will transition to Support at Home with discounted contribution arrangements.

 

Please note:

  • Cancelled visits with less than 24 hours notice will be charged at full cost of the service.
  • Additional and specialised services can be provided, rates to be agreed.
  • Consumables ordered for/by the client will be an additional charge.
  • If the client chooses a contractor, an administration charge of 10% applies.

Our Care Model

The Legacy Aged Care Community places you at the centre of your care, but also recognises the role of the wider community in keeping you well, including your family and loved ones. We keep you connected to the people and things that matter, ensuring a holistic outcome in reaching your care needs.

Care Management

Care Management is a service provided to you to help you get the most out of your Home Care Package(Support at Home) care and services. This includes helping you to identify and prioritise your needs and goals, set up a schedule of services, find the right workers for you, and monitor your services to make sure you are getting great results. Unfortunately, despite some providers’ high fees, their care management is less effective because the people responsible for looking after you are busy with administrative tasks. It’s not unusual for care managers from some Providers to spend more than 50% of their time on admin due to outdated systems and technology. At Legacy Aged Care, we pride ourselves on the quality of care management that you receive, and we work hard to improve our processes and make sure that our Care Managers are focused on you. We take all the administrative work off our Care managers and absorb this into the cost of our Package Management. This way your Care Manager can be in regular contact with you, your representatives and your workers to make sure things are being done the way you like, and to offer their expert advice. And if anything changes, your Care Manager is always available.

Means Testing

Unless you are a full Pensioner, you may be asked to pay an Income Tested Care Fee (ITCF). The Department of Human Services (Centrelink) will assess your income and let you know how much you need to pay towards your care. Your income is reviewed by Centrelink quarterly. Providers cannot waive or discount this fee. If you haven’t had a means test, Legacy Aged Care can help you with this process.

To apply for a Home Care Package(Support at Home), you need to go through a government body called My Aged Care (MAC). To get started you need to request an assessment. You will be given a MAC referral code.

We can make a referral for an assessment to My Aged Care(https://www.myagedcare.gov.au/) on your behalf and help you to prepare. We can also assist you with interim support servicesthat you may need during the time it takes to complete the application and start receiving services.

If Legacy Aged Care is your preferred supplier, you can let My Aged Care know when requesting your assessment. This means when your package becomes available, we will also be informed and can contact you to start organising your support plan.

My Aged Care will contact you with a time and date for your assessment. During this assessment, you should discuss your needs, situation, and any current care or support services that you are receiving.

It is important for the assessors to understand your capabilities with and without support (formal or informal).

The Aged Care Assessment Team/Service (ACAT/ACAS) will schedule a time to visit you at your home. This assessment will determine if you are eligible for a Home Care Package and which package level best meets your support needs. There are four levels – basic, low-level, intermediate, and high-level.

After your assessment, My Aged Care will send you a letter to let you know if you are eligible to receive a Home Care Package, and what level of funding you’ve been approved for. If eligible, you will be placed on a national waitlist and notified when your package becomes available. Packages can have significant wait times and are prioritised on a needs basis.

 

When you receive your letter, call us, and we can guide you through the next steps. While you are waiting we can assist you with interim support services that you may need.

 

 

Approximately three months before you can access your package, you will get a ‘Readiness Letter’ from My Aged Care, which will include some paperwork you will need to complete. We can help you fill this out and start a discussion about your support in preparation for when you receive your package.

Once you have been allocated a Home Care Package, you will receive a letter outlining that a package has been assigned to you.

You’ll have 56 days from the date you receive your letter to choose a provider, such as Legacy Aged Care, and commence services. An extension of 28 days is available should you require more time.

The final step is to create your care/support plan. This is where we get to know you better, learning about your life, and your preferences. We then work with you to tailor a plan that outlines your goals, and the supports required for you to achieve them. Once this plan is complete, we can schedule your first service!
